Please print out and fill out, in black ink only, a copy of the form requested and bring in to the office on your first visit.

Additionally, please bring MRI’s, X-ray images, Bone density reports (DEXA), and Labs pertinent to your chief complaint. For example: if you are being seen for neck pain it is VERY IMPORTANT that you bring your prior MRI of the neck but less important to bring an MRI of your foot. If you are uncertain then please bring all records and we will sort through them.
